High Complexity Care Team

The High-Complexity Care Team in Victoria, BC, is seeking applicants for a long-term Family Physician, Part-time or Full Time. Flexible schedule.

The High-Complexity Care Team (HCCT) is a physician-lead initiative that has been serving the medically and socially most complex patients in the Western Communities since 2019. It is a multidisciplinary organization including FPs, NP, SW, RN, Pharmacy and Psych Consult Liaison. In collaboration with the Victoria Primary Care Network (PCN), Ministry of Health, Island Health and Divisions of Family Practice the HCCT has expanded its successful model into Victoria. We are looking for Family Physicians interested in innovative primary care to join our team.

• Charting/admin/labs time all compensated.

What we are looking for:

• Dedicated, compassionate and collaborative physicians
• Ability to work as a team with Nursing, NP and Allied health in a non-hierarchical manner
• Open to applicants in all career stages, from new grad to semi-retirement
• Open to physicians currently practicing as FPs, ERPs or Hospitalists, but hospital-based applicants must have recent community practice experience per CPSBC guidelines
• We value quality of care over speed/efficiency (but some of both is nice)
• Shared philosophy of trauma-informed, culturally-safe care essential
• Either interest in or experience with mental health and addictions
• Experience in hospitalist, rural or ER would also be considered assets but not required
• Interest in innovation in primary care also highly valued
• Must have valid driver’s license and own vehicle for home visits
