Continued Patient Attachment Through the Health Connect Registry (HCR)
The Health Connect Registry is the centralized and equitable pathway for SSI residents seeking a primary care provider. The total number of SSI residents attached over the past year is 1092. Clinics and physicians use HCR for all new patient attachments, ensuring a coordinated and transparent process for the community.
Strengthening the Local Physician Team
Salt Spring Island’s physician workforce remains stable and is steadily growing:
- Our newest physician continues to build their practice, welcoming patients from the Health Connect Registry.
- The incoming physician planned for September 2026 remains on track and is actively preparing for their transition.
- Strong prospects of an additional physician arriving next fall reflect strong momentum in recruitment.
Supporting Continuity of Care
The successful transition of a long‑standing patient panel to a new physician earlier this year has helped maintain continuity for many island residents. This handover is an example of how collaborative planning supports both providers and patients through periods of change.
