A Registered Nurse works with you and your patient to:

Design a plan to manage their health

Monitor their symptoms and measurements and look for changes

Make sure patients are on track to meet their health goals

Send you reports as needed

Connect with patients through video or telephone so that they receive support from the comfort of their home
Equipment to monitor your patient’s health will be loaned at no cost.
