Self-Management BC offers FREE health programs to adults in B.C. living with one or more ongoing health conditions.
Please note that workshops are scheduled on an ongoing basis throughout the year, so if you do not see an in-person workshop scheduled in your community, please check back regularly for newly added workshops. They will also continue to offer virtual, self-study and telephone-based, and online program options for those who wish to participate from the comfort of their home.
Below is a brief description of all remote program options:
- Virtual Self-Management Workshops- available for Chronic Conditions, Chronic Pain, Diabetes, and Cancer: These 6-week workshops follow the same structure and content as an in-person workshops, but are conducted virtually, via Zoom. Groups of up to 8 participants and 2 Program Leaders meet virtually, once per week, for 2.5 hours per session. Visit the website to view upcoming workshop dates.
- Self-Management Health Coach Program: This is a one-on-one telephone-based coaching program to support people living with chronic conditions to become better self-managers. Each individual is paired with a coach and receives a weekly, 30-minute phone call for a period of 3 months. Coaches are volunteers who may be living with chronic health conditions, and are passionate to help others to achieve wellness. selfmanagementbc.ca/healthcoachprogram
- New! Frailty Coaching Program: This is a one-on-one telephone-based program that supports adults in BC who wish to prevent and/or slow down the progression of frailty. A trained peer frailty coach connects with you by telephone once a week for approximately 30-45 minutes for 3 months. Participants will learn self-management strategies and how to follow the Canadian Frailty Network’s AVOID strategy. Key topics are: activity, nutrition, medication management, vaccinations, and social engagement. https://www.selfmanagementbc.ca/frailtycoachprogram
- Better Choices, Better Health® Online Program: This group workshop is the online version of the Chronic Disease Self-Management Program. Interactive sessions are posted each week for a period of 6 weeks, and participants may log on at their convenience to complete weekly activities and interact with their group via internal messaging and discussion boards. Participants can remain anonymous and everything takes place on a dedicated, secure website.
- Tool Kit for Active Living Program- available for Chronic Conditions, Chronic Pain, Diabetes, and Cancer: This is a self-study program in which participants receive the “Living a Healthy Life” resource book and self-assessment booklet. Suitable for those who enjoy independent self-paced learning.
