New Island Health Guideline: Discharges from Long-Term Care (LTC) to Community

Feb 4, 2026

Island Health’s LTC Program has introduced a Discharge Guideline to ensure transitions from LTC to community are carefully planned and resident-centred. These discharges remain rare and occur upon resident or substitute decision maker request, or when clinical care needs are reduced. The guideline does not change eligibility or create new pathways, and no increase in discharge frequency is expected. Discharge planning addresses ongoing medical needs, primary care access, and medication management. For questions or more information, please contact LTCCoach@islandhealth.ca.
