Primary Care Network Patient Survey – For Distribution

As part of the South Island PCN evaluation process, we’re asking for your support in sharing a patient survey with those who have accessed primary care services at your clinic. This survey will be open from February 10 – March 10, 2025. To encourage participation, please share the survey poster and survey outline within your clinic.
