South Island PCN Patient Survey

Primary Care Networks (PCNs) are groups of primary care clinics working together to care for their community. They have been created by family doctors, nurse practitioners, health authorities, Indigenous communities, family/caregivers and patient partners, and other community providers. This program is guided by a service plan approved by the BC Ministry of Health that includes expanded team-based care, with the addition of new health providers and services to support patients and local doctors/ nurse practitioners. The South Island PCNs include Saanich Peninsula PCN, Western Communities PCN and Salt Spring Island PCN. They are governed in partnership by the South Island Division of Family Practice, Island Health, and local First Nation communities.

Purpose of the Survey

This survey is to learn about the care you get from family doctors and/or nurse practitioners and team members who have been hired to work with them, for example mental health consultants, social workers, and registered nurses (RNs).
The results will be combined into a summary that will be shared with the South Island Primary Care Network Steering Committee, to help understand what is working well, as well as areas for improvement.

Who should complete the survey? 

People who have received care from a patient medical home (family doctor or nurse practitioner) within the South Island region of Vancouver Island.

How will you keep my information safe?

The data is being collected by a research firm Reichert and Associates on behalf of the PCN. We are asking for your consent to participate in this survey. All the information that is shared will be kept confidential and anonymous, and we will not ask any questions that would personally identify you in the survey. There are open-ended questions in this survey. We encourage you not to provide any identifying information, such as names, ages, or identification numbers, in your answers to open ended questions. At the end of the survey, there is an option to provide your name and contact information to participate in a prize draw.
It is important that you know that:
• Participating in the survey is not necessary for you to receive health services.
• All questions are optional. You may stop the survey at any time.
We are collecting information in accordance with the Personal Information and Privacy Act (PIPA).?
