The High-Complexity Care Team (HCCT) is a physician-led initiative that has been serving the medically and socially most complex patients in the Western Communities since 2019. It is a multi-disciplinary organization including family physicians, nurse practitioners, social workers, registered nurses, pharmacists, and a psychiatry consult liaision. In collaboration with the Divisions of Family Practice, the Primary Care Network, Ministry of Health, and Island Health, the HCCT is now expanding its successful model into Victoria.

Key Points:

  • Meaningful work with high-needs, under-served populations.
  • Direct patient care, charting, admin, and lab time compensated at a competitive APP rate.
  • A mix of in-office, home visits, and work from home shifts are available.
  • Option of flexible, parent-friendly scheduling (e.g., 9:30am-2:00pm if desired).
  • Effective team-based care with a shared panel and experienced nurse practitioner, registered nurse, and social worker colleagues.
  • Supportive, fun, and collaborative environment.
  • Daily team meetings for sharing cases.
  • Collaborative relationships with Island Health, Victoria Mental Health, and other community partners.
  • Clinical mentorship from award-winning colleagues, including in psychotherapy skills.

We are looking for another family physician interested in innovative primary care to join our team:

  • Dedicated, compassionate, and collaborative.
  • Ability to work as a team with nurses, nurse practitioners, and allied health professionals in a non-hierarchial manner.
  • Values quality of care over speed and efficiency (but some of both is nice).
  • Shared philosophy of trauma-informed, culturally-safe care is essential.
  • Interest in or experience with mental health and addictions.
