What is an Indigenous Wellness Provider?
The Primary Care Network Indigenous Wellness Provider offers direct support, access, education, and health care planning to Indigenous peoples, while honoring traditional healing practices. They work in collaboration with the PCN Care Team and other healthcare providers to provide a seamless, holistic, and culturally safe approach to health and wellness.
Role Overview
As part of the primary care team, the Indigenous Wellness provider:
- Works with clients and the Care Team to develop and implement client-centered holistic and culturally safe health care plans
- Practices cultural safety and humility, harm reduction, and trauma informed care while honoring traditional healing practices
- Supports mobility and advocacy needs, provides transportation to medical appointments, including accompanying clients into appointments
- Works with clients, families, or other groups to provide education, support, and assistance to access health interventions
- Works with community agencies to promote their services for Indigenous peoples
Wisdom, Training, and Experience
- A diploma is social work, human resources, or a related field, or a combination of education and work experience
- Direct program delivery experience in the community social services sector with a demonstrated working knowledge of community, regional and provincial based programs
- Valid BC’s drivers’ license and access to an adequately insured reliable vehicle
